    Thanks!

My goal is to be able to take the big form and populate a small card that can be cut out, folded, and put in a wallet.

Great form Sean. I lost a friend that was swimming while on the Rubicon. He was traveling with a group of people he knew but he himself was alone in his own rig. He also had a tow rig and trailer at the staging area. We was medivac'd off the trail and was wearing only his swim trunks. His family was down south and it was awhile before everyone else got off the trail and passed info along to his family.

Personally I wear a RoadID (slim) everyday. It's got basic contact info which is fine for most of my instances. After the loss of my friend I made wallet cards and sent a sample out to family and friends so they could carry one. It has personal contact info, say you lost your wallet. It's got medical info as well as vehicle info. Things like a VIN come in handy when ordering parts but also to others who may be looking for you or your vehicle. I keep one in the bag I carry everyday, my wallet, my glovebox and on my bikes.

I carry my wife's taped to the back of mine. Say she were to go missing and the police called and asked for a bunch of useful information, I'd have it. On long bike rides or trips with friends I make sure I have their loved ones info just incase. If traveling in one of their vehicles I've been known to snap a pic with the license plate and email it to my wife or parents.

Having paper in hand is also great to have in case you're without a cell phone or internet.
